Virtual Clinic Process

Your lab test requisitions:

1. Any testing ordered is sent to you by email.

2. Fill in your Alberta Health number, your full name as it appears on your Health Care card, and your Date of Birth before you take the requisition to your appointment.

3. Please look at the form for information on how to book your appointment.

4. The laboratory where you get blood tests and the Radiology place for X-rays/ Ultrasounds offer online booking.

5. Book your appointment.

6. Then use the online calendar above to book your appointment with our doctor to review the test results. Book your appointment 1 week after the date you get your test.
